How to Write a Physical Therapy Appeal Letter
A well-written physical therapy appeal letter answers two questions the payer's reviewer needs answered: was the care medically necessary and does the documentation match what the plan requires. This guide walks through both.
1. Read the denial notice carefully
Every payer denial cites a reason code — medical necessity, lack of prior authorization, exceeded visit limits, missing documentation, coding, or timely filing. Your appeal must respond to the exact reason cited. A generic "please reconsider" letter almost always gets denied a second time.
2. Structure the letter the way reviewers read it
- Header: patient name, DOB, member ID, claim number, dates of service, denial reason.
- Clinical summary: diagnosis, referring provider, onset, prior level of function.

- Treatment summary: CPT codes, frequency, duration, and measurable progress toward goals.

3. Ground medical necessity in functional outcomes
Payers respond to measurable change: gait speed, ROM in degrees, MMT grades, Oswestry or DASH scores, timed-up-and-go, pain on VAS. 5. Attach supporting documentation
- Initial evaluation and any re-evaluations

6. Keep the tone formal and specific
